Ozzy Osbourne‘s official cause of death has been revealed after his July 22 death at the age of 76.

According to a death certificate obtained by The New York Times and The Sun, the former Black Sabbath frontman died of “out of hospital cardiac arrest” and “acute myocardial infarction,” with coronary artery disease and Parkinson’s disease with autonomic dysfunction listed as “joint causes.”

Per The New York Times, Ozzy and wife Sharon Osbourne‘s oldest daughter, Aimee, submitted the death certificate at a registry in London.

Ozzy revealed in January 2020, that he had been diagnosed with Parkinson’s disease, a nervous system disorder that affects movement, in 2003. He also underwent a number of spinal surgeries in recent years after suffering a fall at his Los Angeles home in 2019.

After his death, sources revealed that a “heartbroken” Sharon planned for her husband of over 40 years to spend his final days in England after decades in Los Angeles.

“Sharon had wanted to move back to England for years, especially as she got older. England was always home for them, but it became more than that,” an insider told People. “It was about creating peace and the best life possible together in their final chapter. The path back to England wasn’t easy. His health has been fragile for years… It was a gift, because they got a year and a half together, at home.”

Ozzy died at the couple’s Buckinghamshire mansion. “It is with more sadness than mere words can convey that we have to report that our beloved Ozzy Osbourne has passed away this morning,” Osbourne’s family said in a statement at the time. “He was with his family and surrounded by love. We ask everyone to respect our family privacy at this time.”

He was a father to Aimee, Kelly and Jack with wife Sharon, and also shared older children, Elliot, Jessica and Louis with his first wife, Thelma Riley.

Ozzy was laid to rest on the grounds of his English home after a public funeral procession in his hometown of Birmingham, England, on July 31.
