KUALA LUMPUR: The Selangor Department of Occupational Safety and Health (DOSH) has issued an immediate prohibition notice at a recycling paper mill in Banting, Selangor, after a worker had fallen to his death on Wednesday.

The statement from the director's office of the department said the notice was due to the employer's failure to ensure a safe working system up to the time of the accident.

"Additionally, two improvement notices were issued on the employer's failure to install barriers near the rotating parts of the solid fuel conveyor machine and the failure to implement interlocking and lockout-tagout systems on the machine.
